Personalized Health Partners (PHP) is a medical practice affiliated with CommuniCare Health Services, one of the nation’s largest providers of post-acute care. PHP’s services are currently offered to residents in West Virginia, Ohio, Indiana, Pennsylvania, Maryland and Virginia. Our team of NPs is essential to providing uniquely tailored care that supports each resident’s goals and quality of life.

In addition to competitive rates and benefit packages, our NPs only work first shift, Monday through Friday. They are never on call and enjoy schedules without weekend or holiday coverage. We also offer a 401(k) with employer match and flexible spending accounts.

 Job Duties & Responsibilities

  • Medically manage residents in post-acute settings, including nursing homes, assisted living facilities, residential care facilities, hospice, home sites and telehealth.

  • Provide chronic disease management, conduct assessments, perform medication reviews, and create treatment plans for residents.

  • Review laboratory and other diagnostic test results and conduct interviews with resident/family and interdisciplinary team to develop treatment plans that align with health goals of the resident.

  • Collaborate with primary care physicians, facility staff and resident families.

  • Assume a leadership role within the multi-disciplinary team.

  • Identify/anticipate key health issues for residents.

  • Educate facility staff, physicians and administration.

  • Complete required charting in a timely manner.

  • Adhere to all quality assurance initiatives and provisions.

  • Perform all other job duties as assigned.

Qualifications & Experience Requirements

  • Master’s in Nursing

  • Active State License

  • Active DEA Certification

  • 2 years of experience preferred
